Hetrochromia Iridis
Here you see, what is striking in this picture? Hmmm, two iris of different colors! Yeah, I had read about this condition called hetrochromia iridis in Ophthalmology text but had never a chance to see myself. Few days back I was sitting in dermatology OPD and there this boy came, 12 years old, with his mother. His elder brother had some skin disease but this boy got more attention than the patient, and he should get, right? How many such people have you come across? Hardly few if you have been lucky.
Hetrochromia iridis was congenital (thought to be autosomal dominant) in this case. Though the condition in itself is harmless but it may present in association with few other diseases like-
- Congenital Horner's syndrome.
- Waardenburg's syndrome: Syndrome in which heterochromia presents as a bilateral iris hypochromia in some cases.
- Piebaldism: Similar to Waardenburg's syndrome, a rare disorder of melanocyte development characterized by a white forelock and multiple symmetrical hypopigmented or depigmented macules.
- Hirschsprung's disease.
- Incontinentia pigmenti.
- Parry-Romberg syndrome.
Luckily the boy was perfectly normal and was happy to have his photo clicked in a hospital where children are given injections mercilessly; as is the image of hospital in their innocent minds.

Ainhum
Working in a government hospital has its own charm. Surprised to hear that? Well, I mean managing the cases with limited drugs and instruments. I remember how I struggled once disarticulating a gangrenous toe in minor OT. But here you see this typical picture showing a not so common condition called Ainhum which undergoes spontaneous amputation (autoamputation) in a span of few years.
Ainhum, also known as bankokerend, dactylolysis spontanea or sukhapakla is a painful fibrous constriction usually bilateral and involving base of the fifth toe. The disease is comparatively common among farmers who walk bare foot but the exact etiology is not known.
This patient presented to us with the complaints of pain on walking and wearing shoes. After a detailed interview and examination we found that left 5th toe had already undergone autoamputation 2 years back. Since radiograph showed involvement of bone, we opted for disarticulation of 5th metatarsophalangeal joint and patient was happy to have his symptoms relieved.

Ovarian Cyst
On table photographs from one of the surgeries I assisted during my internship at CG Hospital Davangere. This 45 years old female presented with C/O mass per abdomen for 1 year with associated h/o dysmenorrhea and menstrual irregularities. There was no h/o fever, bowel disturbances, loss of weight or appetite. USG examination showed a large anechoic cyst in peritoneal cavity. Based on USG and clinical findings diagnoses of mesentric cyst, pancreatic pseudocyst and ovarian cyst were considered but on lapratomy it was found to be an ovarian cyst of 12*15 cm size. Subsequently right sided salpingoophorectomy was performed.

One afternoon, while returning after lunch, I saw this interesting sign. Though never had a craze about photography but this one was just irresistible. Later, the same photo was published in "A Day in the Life of India" a photo collection by Times Group to describe our incredible India in the most interesting way.
